4-axis vs 5-axis: does it matter?

Yes. A lot.

4-axis mills (Z4, N4+, E4) handle zirconia, PMMA, wax, and composite. They can't do undercuts or angled screw channels. For straightforward crown and bridge work, they're perfectly capable.

5-axis mills (E5, K5+, S5, R5) add a tilt axis that enables titanium milling, angled screw channels for implant cases, and more complex geometries. If you're doing implant work or plan to, 5-axis is the way to go.

The price jump from 4 to 5 axes is significant. But so is the revenue from implant cases you'd otherwise have to outsource.

Total cost of ownership
